How to Get the Rotten Winged Sword Insignia in Elden Ring

You can obtain the Rotten Winged Sword Insignia after successfully helping Millicent defeat her sisters. It will occur near the very end of her storyline, with the slain sisters now out of the picture. Only by taking down Amy, Mary, Maureen, and Polyanna will the new Talisman come your way after the familial battle.
Weighing in at about 1.4 lbs, the Rotten Winged Sword Insignia holds the power to raise your attack power with successive hits. The power is elevated through three attack tiers: 6%, 8%, and 13%.
Since Millicent is a vital quest-giving NPC, her questline is considerably lengthy, taking you to multiple spots across the Lands Between. We’ve covered her entire questline in another guide that you can check out. It basically commences from speaking with Gowry at his shack to essentially Elphael, Brace of the Haligtree. Simply progress through her storyline until you encounter the Ulcerated Tree Spirit within Elphael. It’ll be near the Drainage Channel Site of Grace.
Defeat the aforementioned Tree Spirit to then kickstart the sister scrimmage with Millicent. Return to the Tree Spirit’s location and you’ll find two summon signs: one to help Millicent, and the other to fight against her. Fighting Millicent will grant the player Millicent’s Prosthesis, another Talisman that increases Dexterity and also contains a successive attack power boost. After helping Millicent with her family business, be sure to visit her near the pool where you summoned her. Lines of dialogue will be exchanged; exhaust these, reload the area, then return to her spot. You’ll find that the Unalloyed Gold Needle has taken her place.
